JOB SUMMARY: Establishes Family Partnership Agreements with families. Assist families with goal setting and follow-up. Develops service plans for each family as needed. Coordinates services to provide assistance, information, referrals, and follow-up to families. Assist in facilitating parent engagement activities. Coordinates training/workshops for families. Conducts children’s application intake and process applications. Enters information into the data management system. Maintains accurate up-to-date files and provides data and reports as required. Makes periodic home visits as needed.
Minimum General Education: Family Development Credential (FDC) or an Associate’s Degree or higher in Social Work, Human Services, or a related social science field.
Minimum Work Experience: Two years of experience in family services or equivalent experience in social services or social science field preferred.
Other Qualifications: Previous case management experience and knowledge of available community resources desired. Must have excellent verbal and written communication skills and the ability to plan and work independently. Must have the ability to perform basic math computations. Must have the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists; to interpret and process a variety of instructions furnished in written, oral, diagram, or schedule form. Must be organized and plan work effectively. Must have a valid South Carolina driver’s license and an acceptable driving record. Must frequently sit or walk. Individuals must be physically capable of assisting in the center(s) as needed and attending community meetings and events. He/she must be able to lift and/or move up to 50 pounds.
