SIGN-ON BONUS UP TO $1500 
(Paid to eligible employees after 6 months of employment)

JOB SUMMARY:
  Establishes Family Partnership Agreements with families. Assists families with goal setting. Develops service plans for each family as needed. Coordinates services to provide assistance, information, referrals, and follow-ups with families. Assists in facilitating parent engagement activities. Coordinates training/workshops for families. Conducts children’s application intake and processes applications. Enters information into data management system, maintains accurate up-to-date files and provide data reports as required. Makes periodic home visits as needed.

QUALIFICATIONS:  Associate degree in Social Work, Family Development, Human Services or related fields preferred.  Two years of experience in family services or equivalent experience in social services and/or developing and implementing a parent related program preferred.  Must be sensitive to the needs of low-income families and have previous case management experience. Knowledge of available community resources desired. Must have good written and oral communication skills as well as the ability to plan and work independently. A valid South Carolina drivers’ license with an acceptable driving record is required.
