JOB SUMMARY: Implements case management of selected customer households. Mobilizes and coordinates internal and external resources to remove barriers and follow up on active cases. Participates with customers to develop a plan for self-sufficiency. Implements approved work program and perform associated duties and activities. Participates in screening, intake, and other direct service delivery. Maintains a caseload in accordance with ROMA goals. Determines barriers to self-sufficiency set objectives mutually agreed to by customers and evaluate customers' progress to modify plans and services. Performs related duties as assigned.
QUALIFICATIONS: Bachelor's degree in Social Work or related field. Licensed Social Worker or equivalent combination of formal training and related experience preferred. At least two years of experience in social service work desired; experience working with low-income families required. Excellent written and verbal communication skills. Valid SC driver's license, good driving record, and dependable transportation required.
